As an out-of-network provider, you would be responsible for payment-in-full at the time of your session with a check or credit card. If you are interested in pursuing reimbursement from your insurance provider, call the phone number on your insurance card to learn more about your benefits and best way to submit invoices. After every session, an invoice will automatically be generated with the information needed for reimbursement: my NPI (National Provider Number), TIN (Tax Identification Number), CPT code (for medical billing) date of service on and diagnostic code.
